Reach Out for Life Program promotes breast health and assists at-risk populations with obtaining mammograms, which includes providing free mammograms for uninsured women who qualify financially. The general eligibility criteria for the program includes:
There are also other more specialized criteria that would allow someone under the age of 40 to receive a free mammogram, such as if there is family history of breast cancer or you believe you have symptoms of breast cancer such as a lump.
Incentives are available for women who get screened through the Reach Out For Life Program.

You can download a mammogram form to take with you to your scheduled mammogram appointment. Make sure it is signed by the technician and return it to us by fax or mail. Once we receive your completed form, you will receive your $15.00 Gift Card.
